Does the preparer have to be a CPA specifically?

No. A CPA, an enrolled agent, or a licensed tax preparer all qualify. What matters is that someone licensed prepared and signed the statement.

Can WVOE and a P&L be combined on one file?

Yes, where a borrower has both wage and self-employed income. Each source is documented on its own terms and the qualifying income is the total.
